While Katsuki can't say Izuku's not imaginary or anything, he's at least willing to let him talk and see what's up, rather than snapping off more explosions or threatening him with death. Katsuki's imagination of Izuku would probably be more... exaggerated than this, so there's a really good chance he's not the one behind the Izuku before him.

Case in point, if this is a mental attack, there's a shit-ton of "blueprints" on Izuku in Katsuki's memories and thoughts.
